We had boundary changes and a ward dissolved last Sunday... As the ward membership clerk, I did did nothing.... It was all handled at the stake level. I did find something helpful.. You can use the Moved Out and Moved In Reports to see the names of those who were moved out and who was moved in.

We went through this a couple of months ago. The Stake Clerk and Stake Membership clerk can't move the records, but they do get to click a very exciting button called "Implement Boundary Proposal Changes." Once that is done it is up to the Ward clerk or ward membership clerk using Move Records after Boundary Realignment tool in LCR to select the members going to Ward A and then move them, and then the members going to ward B and move them. Using the tool you can move them in large groups but you have to click on each family as it does not auto select them.
CAUTION: have the ward clerk move themselves last -- once they move themselves as far as LCR is concerned they are released and can't see or do anything!!! If you do this the stake clerk needs to put someone into the ward clerk calling and get seconded in order to continue the process.

And just to stress, the clerks need to use the right process. If they use the normal moveout process, they're likely to blank out the member's address and whatnot. Using the boundary realignment tool is quicker and keeps the information intact.

And the stake clerk can get a helpful report from the boundary proposal that shows who's moving from which ward to which ward. If they give that to the ward clerks it makes the job easy for those clerks.
